Welcome to the Healthcare table! Family size is needed at this table. Healthcare includes the amount of coverage received
when visiting a physician for an expected or unexpected medical treatment. All family members must be covered by an
insurance plan. There are two options for healthcare, Basic vs. Premium Coverage.

Basic Coverage includes: Preventative care only.
Well visits to your doctor, yearly vaccinations
including flu shot and generic medications. All
other medical care will require out of pocket cost
for procedures andbrand name medications. Basic Basic
Vision and dental are NOT included. Individual plus Individual plus
1 dependent 2 dependents
Premium Coverage includes: Preventative and
sick visits for expected and non-expected medical $100 per month $200 per month
care. All vaccines and flu shots included with no out
of pocket cost to you. Included are generic or brand Basic

name medications and vision and dental insurance. Individual Plus 3
or more dependents
THINGS TO CONSIDER: $300 per month

• Children get sick and hurt often
• Not all medications have a generic alternative

• Brand name medications can get costly
• The American Dental Association recommends
that individuals have dental cleanings at least Premium Premium
